The Planning and Continuum of Care Committees have joined forces to create a dynamic powerhouse focused on shaping a healthier and more supportive community.

Committee Overview:

The Planning and Continuum of Care Committees have combined forces to bring you a unified approach to community development. Here's a glimpse into our core responsibilities:

  • Assessing TGA Needs: We delve deep into determining the size and demographic characteristics of individuals with HIV/AIDS within the TGA. Understanding the community is the first step towards impactful change.

  • Identifying Service Needs and Gaps: Uncover how we identify and address the service needs and gaps of People Living With HIV (PLWH) in the TGA. Our mission is to bridge these gaps and enhance the quality of care.

  • Community Priorities: We establish robust methods to gather information on community needs and priorities. Your voice shapes our initiatives, and together we build a continuum of care that truly caters to the community.

  • Guiding Continuum of Care: Taking the lead in guiding, coordinating, and facilitating the development and strengthening of the TGA's continuum of care. We are the driving force behind ensuring a seamless and comprehensive network of support.

  • Evaluation Leadership: We take charge of evaluating Council products and actions. Accountability is key to our success, and we actively ensure that our initiatives align with the community's evolving needs.
